Your first visit to Center for Cosmetic and Clinical Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. Insurance and Billing

For clinical dermatology procedures (non-cosmetic), we participate with the following insurance plans:

Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. When you call our office for an appointment, please have your insurance information available. If your insurance company requires a referral from your primary care doctor, please tell the receptionist and bring the completed form with you on the day of your appointment. Also, please bring a photo ID, such as a driver's license, with you to your appointment.

If we do not participate with your insurance plan, you will be required to pay in full at the time of service. We will be happy to provide you with the necessary forms needed to submit for reimbursement.

We request that you give us 24 hours notice when canceling your appointment. There is a $50.00 charge for regular (15 min) appointments missed without 24 hours notice and a $100.00 charge for extended (30 min) appointments missed without 24 hours notice.

Payment Options

We kindly request that all co-pays, deductibles, office visits (unless we participate with your insurance plan), cosmetic procedure fees and dermatology product fees be paid at the time of service. We accept checks, cash or credit cards.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
